Step 2. Completing the Identification Request Form

 

Identification Request Forms (IRF) can be completed in two ways:

Online: Performed electronically through our website. Online submission are available only for AccGENX-ID™ and Ribotyping services designated for our Newark, Delaware facility. Online submissions are the preferred method because they are easier and more convenient than hand written forms.

Offline: Identification Request Forms are available in a writable PDF format in which you may also save information. There are specific forms based on the service method and whether you are shipping to our Newark, Delaware facility or our European site. You may either complete the information electronically or print it to write in your information. We encourage you to complete the information electronically to avoid potential delays that occur when hand writing is not clear. Choose the appropriate manual form from the list below.

For either method, be sure to print out the final IRF, authorize the submission by signing the document, and include a copy of the document with the shipment of samples.

Test Codes

On the IRF you must select the appropriate test code for your sample. All test codes are composed of a service package test code and a chosen turnaround time (TAT). TATs are based on business days. Click here for explanation guides of TATs.

Important points to consider for the Identification Request Form:

  • Before you send in your samples, remember to sign the IRF.
  • If submitting plates, remember to properly indicate which colony type(s) you want to ID, and circle them on the plates:
    • If more than one colony type is present, and you would like only the dominant colony type identified, please check the appropriate box.
    • Should you require all morphologically distinct colony types identified, please indicate this (note: subculturing fees will be applied if necessary).
    • If you would like only one type of colony identified, please circle the colony of interest with a permanent marker on the sample lid and dish (mark the side of the plate for alignment purposes), and check the appropriate box.
    • Accugenix will default to “ID Dominant Colony Only” in cases where no box is checked.
    • If necessary, please provide any specific information in the “Comments” column.
    • Insert a copy of the signed IRF with your sample shipment.
