The first step in submitting samples to Accugenix is to complete an Identification Request form.

Identification Request Forms can be created in 2 ways:

  1. Online. This is done electronically through our website. It is easy to use and is the preferred method.

    -OR-

  2. Offline. An ID Request Form is available in a writable PDF format. You may either complete this electronically or print it to write in your information. We encourage you to complete this electronically if possible to avoid any delays that occur with hand writing that is not clear.

For either method, be sure to print out the final ID Request form, authorize the submission by signing the document, and include a copy of the document with the shipment of samples.

Test Codes

All test codes are comprised of a test package and a chosen turnaround time (TAT). TATs are based on business days.

BacSeq - Bacterial samples submitted for Microbial Identification by DNA sequencing. (16S rRNA gene, 500 bp sequencing)

FunITS - Mold and yeast samples submitted for Microbial Identification by DNA sequencing. (Internal Transcribed Spacer (ITS) region)

FulSeq- Samples submitted for Full Gene Microbial Identification (16S rRNA Full gene sequencing)

BacRib - Bacterial samples submitted for Microbial Characterization by automated ribotyping (analyses with 2 enzymes)

BacRibSE - Bacterial samples submitted for Microbial Characterization by automated ribotyping (analysis with one enzyme)

GS - Samples submitted for Gram Stain (1 Day and 5 Day TAT only)

FunMic - Mold samples submitted for a microscopic identification (5 day TAT only)
