Please complete all requested information.
The online ID Request Form generator allows you to import your sample codes from another file (for example: Word, Excel, plain text, etc.) or they can be individually typed.
There is a 42-character limit for each sample code. This is to ensure that we will be able to read the entire code when the form is generated. If this does not meet your requirements please contact Technical Support for further instructions.
Please note that our website is not secure. If choosing the credit card payment option, an Accugenix Technical Support representative will contact you by telephone when the samples arrive to obtain all the necessary credit card information.
After entering and reviewing all the required demographic and sample information, an official ID Request Form (PDF) is generated through our web site. The form is automatically sent to Accugenix to alert us about your incoming order. A confirmation copy is also sent to you by email. Please review the confirmation document for accuracy. Contact Technical Support as soon as possible if your testing needs change after submitting the ID Request Form.
Please print out the confirmation document, sign it, and include a copy with the shipment of your samples.
