Instructions
Please complete all requested information. After entering and reviewing all the required demographic and sample information, review the identification request form for accuracy. Please print out the final document, sign it, and include a copy with the shipment of your samples.
Please note that the writable PDF version of the IRF will not allow you to electronically fill in credit card information for payment. This is to prevent the transfer of sensitive information by email. You may either hand write this information on the hard copy that you include with your shipment of samples, call a Technical Support representative with the information, or we will contact you by telephone when the samples arrive to obtain the necessary information.
The ID request document is in Portable Document Format (PDF) to retain the original format. To view or print these documents, you must use the Adobe Acrobat Reader, an application already available on most computers. The Adobe Acrobat Reader allows you to view, navigate, and print PDF files across all major computing platforms. Acrobat Reader is free and available with installation instructions from Adobe's web site.
